Supported accommodation

Refer for Ryrie Street or Charlemont

We consider the person's goals, preferred routine, assessed needs, household compatibility, current funding and consent before discussing a place.

General support is available from 8:00am–8:00pm, seven days a week. Direct support is individually structured around each person's goals, routine, assessed needs and approved funding, and may be delivered in agreed morning or afternoon blocks.

Support coordinators & local area coordinators

You are matching a person's goals to a living or community support arrangement.

  • How our support model fits the person's stated goals
  • Whether the funding in their plan aligns with the supports being considered
  • Compatibility with people already living in a home, where relevant
  • Timely follow-up: we acknowledge referrals within two business days

Hospital & discharge teams, including Barwon Health teams

You are planning a safe, unhurried transition out of hospital or short-term care.

  • Consent from the person or their decision maker before we are contacted
  • Indicative timeframes and any date pressures you are working to
  • Continuity: who stays involved after the move, and what handover is needed
  • For early triage: goals, current living situation, support needs at a high level, funding arrangement

Community, child/family & government services, including Meli and DFFH teams

You are coordinating around safeguarding, care arrangements or family circumstances.

  • Safeguarding considerations and any current safety plans
  • Who holds consent and decision-making authority
  • Coordinated planning with other services already involved
  • Clear responsibility: who does what, and who we contact first

Secondary schools & wellbeing teams

You are exploring age-appropriate youth program options for a student.

  • Which youth program suits the young person's age and interests
  • Family or guardian consent before an enquiry is made
  • Transition support around school, holidays and routine changes
  • Safeguarding and child safety expectations we work to

Referral pathway

Five steps, clearly documented

  1. Submit

    Send the referral form with a short summary of goals and needs.

  2. Acknowledge

    We confirm receipt and clarify anything missing, usually within two business days.

  3. Assess

    Suitability, compatibility, risk and funding are reviewed with consent.

  4. Meet

    A tour or meeting is arranged with the person and their supporters.

  5. Confirm

    Supports, charges and agreements are documented before commencement.

How quickly will I hear back about a referral?

We aim to acknowledge referrals within two business days and provide an initial view on suitability shortly after, depending on the information available.

What information should I include?

A short summary of goals, current living situation, support needs and funding arrangements is enough to start. Detailed clinical or behavioural information is requested later, with consent.

Do you need consent before we refer?

Yes. Please confirm the person or their decision maker is aware of the referral and consents to Solara being contacted before submitting.

Can I upload documents securely?

Yes. The referral form includes an optional secure upload for referral summaries, service requests or plan extracts. Uploads are private and accessible only to authorised Solara staff.
